PLAN TYPES
Which Plans Are Most Likely to Cover International Care?
Self-Funded Employer Plans
HighERISA self-funded plans have broad discretion to design benefits that include international care. Many large employers have already added medical travel benefits. Your employer can add Acules as a benefit tier without restructuring the plan.
PPO Plans (Out-of-Network)
SometimesSome PPO plans include out-of-network coverage that may extend internationally. Reimbursement is typically based on a percentage of billed charges (after deductible and coinsurance). Check your Summary of Benefits for international coverage language.
HMO / Managed Care Plans
Generally NoHMO and tightly managed care plans typically do not cover elective international procedures. Coverage may exist for emergency care abroad. Acules is often used as a self-pay option alongside HMO coverage.
ACA Marketplace Plans
Generally NoACA individual marketplace plans are generally limited to US providers and do not cover elective international procedures. Acules is typically used as a cost-effective self-pay alternative for patients on marketplace plans.

How to Check Your Coverage Before You Go
Review Your Summary of Benefits (SBC)
Your insurance carrier is required to provide a Summary of Benefits and Coverage. Look for sections on "out-of-network" coverage and any international exclusions or inclusions.
Call Your Plan Administrator
Call the number on your insurance card and ask specifically: "Does my plan cover elective surgical procedures performed at internationally accredited hospitals outside the US?"
Request Pre-Authorization if Applicable
If your plan offers any international coverage, request pre-authorization in writing before travel. Get the authorization reference number. Document all communications.
Understand Your Reimbursement Basis
If covered, understand whether reimbursement is based on billed charges, a fee schedule, or a percentage of "usual and customary" rates. International hospitals may bill differently than US facilities.
Obtain Itemized Bills and Documentation
After your procedure, obtain an itemized bill and a procedure report in English from the international hospital. These are required for any insurance claim submission.
